The Sanguinesti staff is a powered staff requiring 82 Magic to wield, and is only available as a rare drop from the Theatre of Blood. Named after the Sanguinesti region, the staff was crafted by Lord Lowerniel Drakan himself and given to Lady Verzik Vitur as a gift for her assistance in the Misthalin - Morytania War.

The staff has a built-in magic spell that can be used regardless of the spellbook the player is using and cannot be used to autocast any other combat spells. The spell gives no base experience and grants the player 2 Magic experience per damage dealt.

In addition, the spell possesses a healing passive effect in which successful hits have a 1/6 chance of healing the wielder half the amount of hitpoints dealt to a target. This healing effect can be seen via the 'upwards spiral' on the enemy; all other animations are the same.

The spell cannot be used against other players in the Wilderness. The only way that the spell can be used against other players is during the Castle Wars, Soul Wars and TzHaar Fight Pit minigames, as well as the Clan Wars minigame if the clan leaders enable it.

The Sanguinesti staff can be combined with a holy ornament kit to make a Saradomin-themed holy Sanguinesti staff. This cosmetic enhancement won't change the stats in any way.

Charging[edit | edit source]

In order to use the staff's magic spell, it must be charged with blood runes, holding up to 20,000 charges when fully charged. Each cast requires 3 blood runes, costing 1,065 coins per cast. The total cost to fully charge the staff with 60,000 blood runes is 21,300,000. Assuming non-stop use, with one cast every 2.4 seconds equaling 1,500 casts per hour, the staff costs 1,597,500 per hour to use.

Players can uncharge the staff whenever and wherever they choose, getting all of the blood runes back.

Maximum hit[edit | edit source]

The maximum hit dealt by the staff is dependent on the player's current Magic level, starting at 26 with level 82 Magic and increasing by one every three Magic levels, up to a maximum of 39 at level 120 (obtainable only through an overload (+) in Chambers of Xeric). Both magic damage bonuses and stat boosts will modify the max hit of the spell; likewise, stat-reductions will lower the damage. The staff always has a max hit of at least 5, even when reduced to 0 magic. Compared to the other two powered staves, the Trident of the seas has a lower base maximum hit by 4 (max of 20 at level 75), and the Trident of the swamp has a lower base maximum hit by 1 (max of 24 at level 78).

When on a slayer task and using the Slayer helmet (i), the maximum hit is increased by a further 15%. This bonus stacks multiplicatively on top of the other magic bonuses above, but it does not stack with a salve amulet(ei).

A player attack triggers the staff's passive effect.

Compared to the trident of the swamp, the built-in spell hits one damage higher with a maximum base damage of 34 compared to the trident of the swamp's 33. The trident possesses the ability to envenom, while the staff possesses the healing passive effect.

Thus, the staff is most optimal in situations where venom or poison has no benefit and where healing is required. However, if the player's target is not immune to venom, players could initially use the trident of the swamp against the target until the target has been envenomed, then switch to the Sanguinesti staff. Notable encounters where the staff's healing capability proves very useful include the Kraken, the Chambers of Xeric, Dagannoth Rex, Zulrah, and Nightmare.
