The Secluded Chapel is an old ruin found outside Shayzien, south of the Giant Pit.
Inside the chapel spawn 1 coin, 1 iron nail, and an old note. The altar slab cannot be used to restore Prayer points. There is a small graveyard, and the grave digger can be found just to the west.
